Reports To: Operations Director
Grade & Classification: Exempt
$1,000.00 sign on bonus for eligible candidates. Eligible candidates that pass a background check & complete 90 days of employment will receive $500.00, and and additional $500.00 after 180 days.
This position ensures appropriate and meaningful habilitation services for developmentally disabled individuals receiving support from Direct Support Professionals. The scope and sequence of the habilitation program is developed and incorporated into the plan of care by an interdisciplinary team. The team may include the client, family, Residence Manager, Lead DSP, Direct Support Professionals and others who may help identify the client’s needs and design appropriate services.
Must possess the ability to work effectively for and with individuals with developmental disabilities and have the following skills:
Be able to work effectively for and with individuals who have intellectual and behavioral differences.
Effectively support service recipients to develop a community network in their community.
Be able to act independently and function as part of an overall team.
Keep accurate service delivery records.
Good judgment and reasoning skills a must.
Good communication skills and a willingness to listen.
Approved Criminal History Check by the Alaska Background Check Unit.
Current TB test, CPR/First Aid Certification.
At least 21 years of age.
Ability to speak, read and write in English.
Proven organizational skills, prioritizing tasks and time management skills.
Internet skills and basic use of Word and other applications. Will train.
Use smartphones/tablets and installed apps. Will train.
Ability to complete basic arithmetic computations and perform tasks such as balancing a checkbook and recording expenditures.
Ability to follow oral and written instructions.
Knowledge of community resources.
Ability to work flexible hours, including evenings, weekends, overnights, and holidays.
Baccalaureate or higher degree in a related field or at least two years of documented experience, relevant to the population of residents to be served, as a care provider, with documented skills or training relevant to the population of residents to be served.
Applicants must also have:
AK driver’s license and clean driving record.
Reliable automobile (See policy manual).
Current Auto Insurance, including liability – minimum suggested $300,000.
Duties (Specific for each individual)
A primary duty of a Team Care Manager is to supervise the team to enhance the ability of the service recipient to lead a self-determining life by providing administrative support and information necessary to build self-esteem and assertiveness to make decisions. To accomplish these duties, the Team Care Manager will understand the service recipient’s preferences, develop their POC and be oriented to the goals and objectives for respective service recipients. The Team Care Manager will facilitate and teach Team Leaders and Resident Managers to skills necessary to ensure service utilization, health and safety oversite, team management and support. A Team Care Manager may be required to complete direct service responsibilities in accordance with DSP and Crossroads standards of care. Additionally, the Team Care Manager oversees some management operations to include auditing paperwork, scheduling, modeling skills, training DSPs, staff supervision, after hours and on-call responsibilities.
Specific duties include:
· Administrative duties:
o Mentor Team Leaders, Resident managers and DSP’s in the implementation of training programs for developmentally disabled people receiving habilitation services through the Division of Senior and Disability Services.
o Mentor, model, and ensure adherence to CMS final settings rules.
o Ensure implementation of care plan by training DSP’s in the use of current and correct training techniques.
o Ongoing contact and coordination with families’/ guardians regarding services as needed.
o Directly supervise Team Leaders, Resident Managers and DSP’s and monitor performance.
o Observe and direct safety precautions and security procedures.
o Participate in Behavioral Planning with specialists, completed documenting as required on Behavior plans and communicate with specialists as outlined in behavioral plans.
o Keep a variety of records and compile reports concerning resident training, care and treatment as needed.
o Coordinate with Training Director to ensure required DSP trainings are current.
o Primary point of contact with the Director of Operations and the Executive Director.
o Interview and hire DSP’s that have characteristics outlined in the Code of Ethics and have the potential to become long term employees.
o Complete staff scheduling.
§ Ensure schedules are current in Therap at all times.
§ Assist with scheduling leave.
§ Assist with finding coverage for call-outs, as needed.
· Ensure cleaning standards according to 7 AAC 10.1040 are maintained at all times.
· Ensure the homes maintains Universal Precautions as outlined in 7 AAC 10.1045.
· Ensure Caregiver Hygiene standards are met according to 7 AAC 10.1050 at all times.
· Ensure ASAM standards are complied with at all times.
· Direct Care Duties:
o As needed to ensure the health and safety of all Service Recipients.
Assist people who receive our services to share places and develop a positive reputation and status with people in their community.
Assist service recipients in making healthy and safe choices that affect their life in their community, while respecting their right to choose how they live.
Assist service recipients to grow in their relationships with others in their community.
Provide direct training and related support services to people who receive our services.
Understand and adhere to CMS final settings rules.
All duties as outlined for Team Leader or Resident Manager as needed.
Communicate with the Executive Director or Director of Operations regarding Service Recipient needs.
Admission or discharge from institutional settings.
Significant changes in behavior or health.
Any event that requires police or medical intervention.
Housing or Repair needs
Purchases outside of prior approvals.
· Other duties as agreed upon by the Director of Operations and/or the Executive Director.
